Endocrine and Diabetes Associates, LLC

Updated March 12, 2024

I am excited to announce that I will continue as an Endocrinologist at Comprehensive Primary Care (CPC), 15825 Shady Grove Road, Suite 140, Rockville, MD 20850. Endocrine & Diabetes Associates, LLC will cease as of March 31, 2024.

I will start seeing patients at CPC starting April 15, 2024, ensuring continuity of care. As in the past, I will not be providing services as a primary care provider.

CPC providers have been in Montgomery County for many years and currently have locations in DC & Virginia. Some of the important reasons I decided to collaborate with CPC besides having worked with many of their primary care physicians for many years, is that their focus on patients was the best match to my own ideal of having a ‘health partnership with patients’, focus to provide one on one care, and uniquely tailoring care that matches the distinctive health needs of each patient.

If you already had appointments set up with me for April 15, 2024, and after, please note you will be seen at the CPC Shady Grove office. When calling for an appointment at CPC, please mention that you have seen me prior so that you can be scheduled as an established (follow-up) patient. CPC participates in all major insurances and is in the process of credentialing me appropriately, but please do confirm if you have any concerns CPC is working on transferring medical records for patients I have seen at Endocrine & Diabetes Associates.

Should you need to reach me during this time of transition, you can contact me at Endocrine & Diabetes Associates until March 31, 2024, or by emailingrequest@advanced-endo.net. After April 1, you can reach me at CPC at 301-869-9776If you have any concerns or run into unexpected problems, please let me know.  Please note that this email is not monitored actively so I will not be able to assist with any care related questions.

All medical records from Endocrine & Diabetes Associates, LLC will be transferred to Desert River Solutions after April 2024. Please visit the site below for information on how to request your medical records: www.DesertRiverSolutions.com/request. If you require a copy of your records to transfer care to another provider, you can reach out to Desert River Solutions. To support continuity of care with this transition, and to minimize any record transfer costs to you, Endocrine & Diabetes Associates is also sending a copy of your records to CPC. If you do not want your medical records sent to CPC, you will need to contact [Dr. Shetty] at [ashetty@endocrinemd.net] by March 31, 2024, to inform Endocrine & Diabetes Associates to not share your medical information with CPC.

I hope we can continue the relationship, but if not, for whatever reason, I thank you for the opportunity to take care of you and your loved ones since 2000.

My best wishes for your continued health and happiness.

 Regards,

Dr. Archana Shetty
